All players participating in a Shining Stars event will need to have a Player Registration and Insurance waiver form on file. The forms can be found below. The Player Registration form needs to be submitted online while the Insurance waiver must be printed out and returned to Sonny Scarlett before participation.
The waiver can be scanned and returned via email to sonny@shiningstarsbasketball.com or in person prior to the event.
PLAYERS WILL NOT BE ALLOWED TO PARTICIPATE IN CLINICS, SESSIONS, PRACTICE, GAMES OR TRYOUTS WITHOUT THE COMPLETED FORMS ON FILE.
